  APPLICATION PROCESS                                                                                      

Positions:
Four fellow positions are offered each year.


Eligibility:
To be eligible for one of the four fellowship positions, the applicant must be a board certified
or board eligible graduate of an ACGME accredited residency program in Family Medicine, Internal Medicine, Pediatrics, Emergency Medicine or Physical Medicine and Rehabilitation.

Timeline:
Our fellowship participates in the National Resident Matching Program (NRMP) Specialty Match process.  We adhere to all application, interviewing and match list deadlines specified by the NRMP.  Our deadline for accepting application materials for positions beginning July 2011 is September 30, 2010.

•  Personal Statement

•  Photograph

•  Residency Program Director’s reference letter (must include anticipated graduation date)

•  Reference letters (minimum 2)

•  Copy of current licensure

•  Current C.V.
•  Copies of Dean’s letter & Medical School Transcripts (copies from your residency file)

•  USMLE-attempt disclosure form (see "Apply to Us" or (download form here).

•  Published abstract or title page for all publications listed on application
•  Copy of program for all presentations listed on application
•  International graduates must also provide a valid ECFMG certificate
•  Non US citizens must also provide a valid J1 Visa Certificate or Copy of Green Card

MATCH REGISTRATION:
